Professeur en psychiatrie, Katherine Roshak a tout pour elle : la beauté, l'intelligence et une brillante carrière. Mais soudain, Katherine voit apparaître sa mère qui s'est suicidée quatre ans plus tôt. Effrayée par ces hallucinations de plus en plus menaçantes, elle se rend au cimetière où est enterrée sa mère et découvre que son cercueil est vide. Avec Dominic Delaserra, un flic impulsif, elle cherche à découvrir la vérité tandis que les morts violentes se multiplient autour d'eux.
