Le lieutenant Rachel Scott, chirurgien émérite de l'armée américaine, revient du conflit irakien, où elle a dû gérer des situations souvent très difficiles. Aux Etats-Unis, elle retrouve rapidement un poste dans un hôpital militaire. Mais son caractère bien trempé l'amène à entrer rapidement en conflit avec le système bureaucratique et, surtout, avec sa chef de service, le commandant Helen Keller.
